(44) Examining Advanced Nursing Practice in Two Cities: Hong Kong and Guangzhou, China

There were studies evaluating advanced nursing practice (ANP) independently in Hong Kong and in China, but there was no attempt to make a comparison of practice between them. This study employed a case study method to examine and compare ANP in Hong Kong and Guangzhou.

Purposive sampling method were used to recruit advanced practice nurses (APN) who came from the specialty of medical, surgical and paediatric in Hong Kong and Guangzhou.
Questionnaire and interview were employed for the data collection. The questionnaire included demography, structure, process and outcome. The structure describes the factors influencing how APN roles are implemented. The process describes APN role implementation or the types of services and interventions provided and how they are delivered. The outcome describes the results and service impacts of APN role.

Twenty-four APNs participated in the research. Findings revealed that in the structure domain, APN education & development, healthcare professional team and administration & policy were identified as factors that influenced the development of ANP in both cities. For the process domain, APNs from Hong Kong reported a relatively higher percentage of time spent on direct patient care, while APNs from Guangzhou reported a relatively higher percentage of time spent on researches or project work and meetings. Some of the APNs from both cities could initiate therapy, diagnostic tests and protocol-based medication adjustment. APNs from both cities demonstrated competency as identified in the ANP framework specified in the Hong Kong Academy of Nursing. For the outcome, patient safety, client satisfaction, caring of the career, healthcare professional satisfaction to APN, symptom management, activities of daily living level and complication prevention were identified as key indicators reflecting APNs’ role outcomes.

This study reviewed the dimensions of the roles, outcomes indicators, competency of APNs and factors influencing advanced nursing practices in Hong Kong and Guangzhou.
